Player Registration

Please complete one registration form for each player. 

All families must submit a signed Code of Ethics and Conduct form with their registration forms.  Please take the time to review this Code with each player, parent and spectator who will be visiting any LYS or NCSA function.  Registration forms sent without payment and a signed Code are not going to be processed.  

Coach Applications

LYS is always in need of qualified coaches, especially for the PS/Kdg and 1st & 2nd Grade teams.  Please consider volunteering this year!  The application process is easy...start by reading the Coaches' Code of Ethics and Conduct policy by clicking on the following tab:

Next, complete a coach's application form and sign the Coaches' Code of Ethics Pledge and submit to LYS according to instructions on the form. 

Sponsor Applications (Spring Program Only)

With over 30 teams in the Spring, LYS is always searching for sponsors.  The cost to sponsor a team is $175 and comes with the following benefits:

* Business name printed on team t-shirts

* Business name on LaVilleSoccer sponsor webpage with link, if applicable

* Team photo on engraved plaque.

Compliment & Complaint Form

LYS has established a method to recognize our volunteer coaches, league officials and referees.  We have also established a procedure to register any complaints.  Please see the link below for additional information on either of these topics.
